Referrals and getting started

Do I need a referral?

Yes. We require a letter of medical necessity, or an office note from your physician requesting a neuropsychological assessment. A verbal recommendation is not sufficient — we need it in writing.

Can my physician refer me directly?

Yes. Physicians may fax a letter of medical necessity or an office note requesting a neuropsychological assessment, along with any relevant medical records, directly to our office at (631) 366-2043.

How do I schedule an appointment?

Start by requesting an evaluation on this website. Once your information has been reviewed, our office will contact you about scheduling. If you would rather not use the online form, call (631) 366-3369 and we will take your information over the phone — that is equally welcome.

What happens after I submit my request?

Our clinical staff reviews your information, verifies your insurance where applicable, determines whether this practice is the right provider for your needs, and then contacts you about scheduling.

How soon can I be seen?

Availability varies with the reason for evaluation, insurance authorization, and the schedule. Our office will contact you with the earliest available appointment once your intake has been reviewed. You may also ask to be contacted if an earlier appointment opens up.

About the practice

What is a neuropsychological evaluation?

A comprehensive assessment of thinking and brain functioning. It evaluates memory, attention, language, problem solving, executive functioning, processing speed, visual-spatial skills, and emotional functioning, in order to clarify a diagnosis and produce treatment recommendations.

What conditions do you evaluate?

ADHD, learning disabilities, autism spectrum disorder, memory loss and dementia, traumatic brain injury, concussion, stroke, brain tumors, epilepsy, multiple sclerosis, Parkinson's disease, cognitive decline, medical conditions affecting cognition, and emotional or psychological conditions where cognitive concerns are present.

What ages do you evaluate?

Children from age 7, adolescents, and adults through the geriatric years.

Is Dr. Barnoski board certified?

Yes. Dr. Barnoski is board certified in Clinical Neuropsychology by the American Board of Professional Psychology (ABPP), and has decades of experience evaluating complex neurological and psychological conditions.

Do you provide therapy or counseling?

No. This practice is devoted exclusively to neuropsychological assessment and consultation. We do not provide counseling, psychotherapy, cognitive rehabilitation, neurologic consultation, or psychiatric assessment. If you need one of those, we will tell you and point you in the right direction.

Insurance and payment

Do you accept my insurance?

We participate with several plans, including Medicare, UnitedHealthcare, Oxford, EmblemHealth, HealthFirst, Empire / NYSHIP, GHI, New York State Workers' Compensation, and No-Fault. Insurance participation changes periodically, so our office verifies eligibility before your appointment.

Will you verify my benefits?

Yes. We verify your benefits as a courtesy, and can often do it electronically within seconds. However, benefit verification is not a guarantee of payment. You remain responsible for any deductibles, copayments, coinsurance, or non-covered services determined by your insurance company.

What will it cost me out of pocket?

That depends on your specific plan and benefits. Our office will discuss your estimated responsibility after your benefits have been reviewed, wherever possible. We will not guess at a figure your insurer has not given us.

What if my insurance does not cover testing?

Private-pay options may be available. Our office will discuss the fees with you before anything is scheduled — you will not be surprised by a bill.

When is payment due?

Copayments, deductibles, coinsurance, and private-pay fees are due at the time of your appointment. If you would like to discuss financial arrangements, speak with our billing office.

Do you accept Medicare, Workers' Compensation, or No-Fault?

Yes to all three.

The evaluation

How long does testing take?

Most evaluations involve an interview followed by several hours of testing — typically four to six hours, depending on the referral question. Some are shorter and some longer. Our office gives you a more specific estimate when your appointment is scheduled.

What should I bring?

Photo ID, your insurance card, a list of your current medications (or the bottles themselves), your glasses and hearing aids if you use them, your referral or authorization if applicable, and any records or prior evaluations we have asked for.

Should I stop taking my medications?

No. Continue taking your medications as prescribed, unless your prescribing physician or Dr. Barnoski specifically instructs otherwise.

Can someone come with me?

Yes. A family member or caregiver may accompany you and may be asked to contribute history during the interview. Family members are not present in the room during formal testing.

Will the testing be difficult?

Some parts are challenging by design — they measure your best level of functioning, so they have to get hard. Almost everyone manages the day well, and breaks are provided as needed. Nothing about testing is painful or invasive.

Results and reports

When will I receive my report?

Reports are typically completed within one to two weeks after testing finishes, though timing varies with the complexity of the evaluation. We suggest calling about a week afterwards to check whether it is ready.

Will someone go through the results with me?

Yes. You can either review the results with your referring physician, or schedule a feedback appointment with Dr. Barnoski to go through the findings and recommendations. Results are not given out over the phone by staff.

Will my doctor receive a copy?

Yes. The referring physician receives a report once it is complete.

Can the report be sent to my attorney, school, or employer?

Yes, with your written authorization.

Policies

What is your cancellation policy?

We ask for at least 24 business hours' notice for a consultation, and at least 48 business hours for a comprehensive evaluation, because those appointments reserve several hours of testing time. Late cancellations may be subject to a fee, and repeated late cancellations may affect future scheduling.

What happens if I am late?

Arriving late may shorten your appointment or require it to be rescheduled, depending on the day's schedule. Please call as soon as you know, with an estimated arrival time, so we can work out whether the appointment can still go ahead.

What happens if I miss my appointment?

We maintain a strict no-show policy: an evaluation reserves several hours that cannot be reallocated at short notice. Patients who miss an appointment without notice are generally not rescheduled. If an emergency prevented you attending, tell us and the office will review it.

What should I do if my situation is urgent?

If you are experiencing a medical emergency, call 911 or go to the nearest emergency department. This practice does not provide emergency or crisis services.
