Referring a patient for neuropsychological evaluation.

We welcome referrals from neurologists, primary care physicians, psychiatrists, rehabilitation specialists, attorneys, and other healthcare providers.

What we need

What to send

Please provide a letter of medical necessity or an office note requesting a neuropsychological evaluation. When possible, please include the specific referral question or clinical concern you would like the evaluation to address.

Relevant records are also helpful, including prior neuropsychological evaluations, neuroimaging, neurology or psychiatry notes, and hospital or rehabilitation records. For pediatric referrals, please include relevant school records and any prior IEP or 504 Plan documentation.

  • Traumatic brain injury and post-concussive syndrome
  • Stroke, brain tumor, epilepsy, MS, Parkinson's disease
  • ADHD, learning disability, and autism spectrum evaluation
  • Differentiating cognitive from psychiatric contributions
  • Capacity, fitness for duty, and return-to-work questions
  • Workers' Compensation, No-Fault, and personal injury matters
  • Baseline and serial assessment to track change over time

Not provided by this practice

  • Counseling or psychotherapy
  • Cognitive rehabilitation
  • Neurologic consultation
  • Psychiatric assessment or medication management
  • Emergency or crisis services

Patients age 7 through the geriatric years are seen. We do not evaluate children under 7.

What happens next

From referral to report

  1. 01

    Intake and review

    We collect the patient's history and insurance details, review your referral, and confirm the practice is appropriate for the question asked.

  2. 02

    Benefits verification

    Coverage is verified electronically before scheduling, including whether prior authorization is required.

  3. 03

    Evaluation

    The evaluation begins with a clinical interview with Dr. Barnoski, followed by neuropsychological testing tailored to the referral question. Testing length varies depending on the nature and complexity of the evaluation.

  4. 04

    Report

    A detailed report with findings, diagnostic impressions, and specific recommendations, typically within one to two weeks of testing.

  5. 05

    You receive a copy

    The referring provider receives the report on completion. Release to any other party requires the patient's written authorization.

  6. 06

    Patient feedback

    The patient may review findings with you, or schedule a feedback appointment with Dr. Barnoski directly.

Time-sensitive referrals

If an evaluation is needed to inform a time-sensitive treatment, surgical, rehabilitation, or disposition decision, please indicate this clearly with the referral. Time-sensitive requests will be reviewed promptly, and we will make every effort to accommodate them when clinically appropriate and scheduling permits.
