We participate with the health plans listed below. If your plan is not listed, you may still submit an evaluation request to learn about available options, including private-pay and use of out-of-network benefits.

Insurance participation changes periodically. Our office verifies your eligibility before your appointment, and will tell you plainly if we cannot bill your plan.

How verification works
As part of your online evaluation request, we use the information you provide about your insurance to electronically check the benefits your health plan reports for neuropsychological evaluation.
Insurance companies vary in the information they provide, and some evaluations may require prior authorization or additional documentation. If additional information is needed, our office will follow up with you before scheduling.
- Whether your plan is active and in force
- Your copay for the relevant benefit category
- Your coinsurance percentage
- Your deductible, and how much of it remains
- Whether prior authorization appears to be required
Verification is not a guarantee of payment
This is what your insurance company reports during verification, not a guarantee of payment. Your final responsibility is determined by your plan and by how the claim is processed. Our staff will confirm your exact benefits before your visit.
You remain responsible for any deductibles, copayments, coinsurance, or non-covered services determined by your insurance company. We will never tell you a service is definitely covered, and we will never quote you a figure your insurer did not actually return.

Payment
When payment is due
Copayments, deductibles, coinsurance, and private-pay fees are due at the time of your appointment.
If testing is not covered by your insurance, private-pay options may be available and our office will discuss the fees with you before anything is scheduled. You will not be surprised by a bill.
